5.4 Delivery Procedure

The Company requires documentation of all scheduled deliveries, including the client’s name, address, item to be delivered and any additional needed information. The organization maintains proof of delivery of all home medical equipment and supplies. Receipt and verification of all equipment and services is confirmed with the patient/client by means of a signature.
Delivery requests are filled in a timely, efficient, and accurate manner by qualified personnel. Equipment is delivered to patients as ordered, unless alternative arrangements have been made. Requested items that are not in inventory are secured within a reasonable period of time. The patient/client is notified of any delays that arise.
Rental orders are filled with equipment that is new (when applicable), or used, clean, free from functional defects and relatively free of major cosmetic defects. Delivery staff members are responsible for ensuring that these standards are met when the equipment/device is delivered and set up.
All equipment and devices are labeled properly, handled, transported, and delivered safely, and secured in vehicles appropriately. Equipment is loaded on the delivery vehicle and restrained in a way to prevent unnecessary damage during transit, respecting common infection control practices. Delivery staff takes appropriate care in loading and unloading the equipment to prevent damage to the equipment and injury to the employee, client/patient, or caregiver.
If a patient/client refuses the delivery of a product, equipment/device, or service, the delivery staff explains the consequences of refusal or delivery, and has the patient/client sign a refusal. The supervisor, prescriber, and referral source are informed of the patient/client’s refusal of delivery in a timely manner.
If no one is at home to accept the delivery, this fact is noted on the delivery log and another delivery time is set up, unless the patient/client or caregiver has requested that an alternative arrangement has been made, which is determined to be safe and effective. At no time would an item/device requiring instruction for use be left without such instruction being given to patient/client or caregiver.
Delivery information remains secure at all times and is not communicated to individuals other than appropriate staff members involved in the care of the client. Delivery documentation is kept on file for minimum of seven (7) years.
A daily record of deliveries is maintained, and the following information is logged as needed:
- Name of driver/delivery technician
- Date
- Truck number
- Patient/client name
- Location and map reference
- Type of service
- Order number
- Patient/client telephone number
- Other information as appropriate

State-Specific Regulations
- Arkansas and Florida: At the time of delivery, a schedule is established for any appropriate follow-up services, such as periodic maintenance, supply delivery, and other related activities.
